What is Guardian Medication Review (also known as Medication Check-up)?
Guardian medication review is a patient care service which our Guardian pharmacist examines your medicines with the objective of optimising the impact of your medicines, minimising the number of medication-related problems, reducing waste and drug costs.

Benefits:

Optimizing Treatment Outcome: Getting better effect from your medication e.g. certain medication works better if you take before food.

   •  Identify and minimise side effects due to medication.

   •  Identify and minimise drug interactions.

   •  Simplify ways of taking medication.

   •  Reduce drug cost whenever possible.

   •  Patient education on medication.

Who should undergo medication review?
Patient who is taking on more than 1 medication, health supplements and / or complementary medicines are encouraged to undergo medication review.
(Caregivers are welcome to join the session) 

How much does it cost?
$2 per session

How long is one session?
30 minutes to 1 hour depending on the complexity.

Do I need to bring anything for the review?
Bring all your medication including prescribed and over-the-counter medicines, health supplements for the review.
