Patient Care Centres
Guardian Patient Care Centre is easily accessible by customers. It provides a total patient care programme that is personalised and comprehensive in nature. It is supported by a computerised clinical information system which is linked to a database from the United States and a Patient Care Pharmacist who has more than 3 years of clinical experiences and have gone through intensive training on smoking cessation and weight management.
The Patient Care Centre provides private and conducive counselling area where the customer could have a face to face consultation with the pharmacist in a comfortable sitting area.
Customers will also be able to access a wider range of pharmacy services provided by our trained pharmacist. With our commitment to serving our customers with the widest range of pharmacy services, Guardian has launched a quit smoking programme last year and a weight management service a month ago.
Guardian pharmacists can also assess the customer's risk of having coronary heart disease by conducting screening tests such as lipid profile measurement.
Patient Care Services:
a) Customised Patient Information Data
Our customers receive personalised information about the drug that they are taking: How to take it, when to take it, drug/food to avoid, any side effects that may need immediate attention from a doctor e.t.c.
b) Medication Review
They may bring their accumulated bags of medication from their doctors to the centre where our Patient Care Pharmacist would sort them out and advise them whether the medications are still safe to be taken. Most of all, the medication history and all information provided will be kept confidentially.
c) Counselling
They also bring home with them a comprehensive and easy-to-read literature on smoking cessation, diabetic care, high blood pressure management and weight & cholesterol management so that they are able to be involved in the management of their own medical conditions and practise responsible self-care.
d) Health Screening
They could have their blood pressure, blood cholesterol or blood sugar check by the pharmacist with a minimum charge.
